衛生福利部及健保署積極推動分級醫療，讓民眾能獲得最好的醫療照護。健保署中區業務組鼓勵大醫院與社區醫院與診所合作聯盟，配合健保電子轉診平台及雲端資訊，進一步促進雙向轉診合作的實質效益。中山醫學大學附設醫院日在健保署長李伯璋見證下，與中彰投地區醫院及基層診所代表舉辦「中山大雁、共榮基層」啟航儀式，期盼打造中區優質健康照護大聯盟，推動分級診療及雙向轉診，建立合作互助、資源共享的醫療模式，帶動社區基層醫療發展。
中山醫學大學附設醫院與部立南投醫院等19家地區醫院、250家診所及3家居家護理所簽定策略聯盟，將雙向轉診快捷化，每日保留600個門診名額給轉診病患，轉診中心提供掛號服務單一服務窗口，由專人辦理轉介病患，建立轉診民眾綠色通道，並提供轉診優惠，給予轉診病患完善便捷的醫療照護。
同時推動「雙主治醫師」制度，當病患轉診到醫院時，將即時發送簡訊通知診所，以利原轉介診所醫師能掌握病人就醫動態，並藉由「智慧轉診平台」與醫院主治醫師雙向溝通無縫接軌，有效連結轉診病患回到社區診所的後續醫療照護。
中國醫藥大學附設醫院結合中部地區社區診所簽約合作，成立「健康守護聯盟」，透過健保署醫療資訊雲端查詢系統與轉診平台達到即時雙向溝通，協助急重症到大型醫院，輕症回歸社區診所。
